Malaria is a life-threatening parasitic disease caused by Plasmodium species transmitted through the bite of infected female Anopheles mosquitoes, with P. falciparum being the most dangerous species due to its ability to infect red blood cells of all ages and cause severe complications including cerebral malaria, severe anemia, and multi-organ failure. The clinical distinction between uncomplicated and complicated malaria is critical for medical students to understand, as complicated malaria requires urgent intravenous artesunate treatment and carries significant mortality if not promptly recognized through features such as impaired consciousness, respiratory distress, hypoglycemia, or parasitemia exceeding 10% of red blood cells. Mastering the pathophysiology of microvascular sequestration, the metabolic derangements, and the species-specific clinical patterns enables appropriate diagnostic testing with peripheral blood smears and rapid diagnostic tests, while also informing treatment decisions with artemisinin-based combination therapies.
